Position:
Part-Time Case Manager (20 hours/week)
The Case Manager supports direct service delivery by providing comprehensive, ongoing case management services. Responsibilities include client assessments, individualized service planning, resource coordination, follow-up to ensure continuity of care, and assisting with caseload demands.
Timeframe:

Position Ends: February 26, 2027
Pay: $22 per hour
